They have sections where you can fill out what your username is, your e-mail address, when the hacking occurred (which was sometime time in the last 24 hours), give a list of friends you have, the usernames of anyone else who uses your computer, any other accounts you have, any missing items, and how you were hacked (they mentioned something about the fact that you changed your password). Fill these out as best as you can.
