I found this tutorial on Google Images. The grommets will be challenging, so make sure to use a thick enough fabric (or two layers of fabric), so they do not tear themselves out.
The directions are in Japanese, and unfortunately, I cannot read them. But the diagrams are pretty nice, however.

And here is the pattern that I tried to resize to normal document increments. If you do print it, though, you might check to make sure that the page is set as large as you can allow.
