At the start of your second play-through, look near where the logs are on the path. There should be a chainsaw. Yes a chainsaw. It does massive damage. Another thing you must do is have the first play-through game on the same memory card. Saavy? ^^

If you want to first play the game through for plot (save the challenge for later), or you are scared out of your mind playing, Choose easiest action level. You can't die. You can stand in front of pyramid head for hours and let him swing his giant sword at you and you will not die :3
(so? I'm a wimp! Doesn't mean I love the game any less razz I only used it first time through anyway)

When you beat the game once, go to the options menu *either on the main menu, or during the game* and press R1 + L1 to open the hidden options menu. This menu has 2 features I personally take note of.... The ability to change blood from normal red to black, green, or violet. You can also changed the ammo effect allowing you to recieve normal, X2, or X3 ammo that you pick up.
Also, about the pyramid head swinging his sword at you thing..... Pyramid Head has 1 move with the sword that kills you in 1 hit no matter which difficulty you are on. When he does his overhead slash, make sure you dodge it by either running to the opposite side of the room, or by using your L1 or R1 button.
I'm glad someone posted the chainsaw tip, it's always cool biggrin
This is sort of common sense, but while playing the game, make sure you keep a close eye on Jame's head. If his head turns to look at something, make sure you rotate him to see what it is. This is a great way to find more ammo and health items or possibly find important items needed to proceed in the game. *reason for listing this tip is because most people get too caught up in running, or get so jumpy that they don't pay attention to the little things like that*.
To better improve your star rating, save once every 3 save points *unless you have already beat it once, then only save right before a boss fight*.
Another helpful tip, check every door and location in an area. Sometimes you get bummed out on a good deal when the room is empty or is only filled with monsters, but sometimes you get lucky and find a room with ammo or health drinks. The same issue comes with Pyramid Head's sword. I can't name the number of people who pass The Great Knife's location everytime they play because they get in a hurry to find the true way out. I kinda pity them because Pyramid Head's sword kills any basic monster in 1 hit *except for doormen.... i tried it and the sword kept missing.... better to use a shotgun or hunting rifle on these things*
While fighting the hanging demons in the hospital, if one of them starts to strangle you, mash buttons and rotate analog sticks to be released quicker.
Your flashlight can and WILL die over a long period of time. Make sure you NEVER leave it on while in a well lit area. Though we ask you not strain your eyes, even in dark places, rooms and halls are luminated to a degree and monsters are very illuminated. Also, some TV's have built in brightness modes. *mine has a game mode and a video mode. video mode is really bright, game mode is just the right brightness depending on the game*. If your flashlight dies, and you MUST have a light, simply check the brightness level of your tv or the mode it is set on.
