• In this settlement, named Philly after the ruins it was built on, Smitty became a respected figure within the community, with a reputation of thoroughness and austerity.
• However, his life in Philly didn't last for as long as he'd hoped. Ever since a band of super mutants were found near the settlement's perimeters, nothing stayed the same. One of the lead scientists of the community had went missing, until Smitty spotted him a few days later, hooded and on edge. He followed after the man with at first concern, then suspicion. From underneath his hood, Smitty could see something was off about his face, as though he were undergoing some kind of horrible mutation. Realizing that he was attempting to make off with the secured FEV samples confiscated from the mutants weeks ago, Smitty tried to stop him, but to no avail. The last thing he remembered before being knocked out was having his face splashed with a thick, green goo.
• Upon gaining consciousness the next day, he was too late to warn everyone of the threat in time, and the effects of the contamination were beginning to present themselves within the other citizens who had consumed the water. Despite not having been in contact with the contaminated water supply, Smitty was showing more advanced stages of mutation than they had, likely from the attack the night prior.
• Along with a majority of the other citizens, Smitty's mutation progressed until he was unrecognizable, physically and mentally. Either from some effect of the FEV on his brain, the head trauma he suffered, or a combination of the two, Smitty's personality swayed from uptight doctor to rambling lunatic. However, he still retained all his memories, though seemed to place no importance in it or his former identity.
• Due to his obvious mutations, Smitty was driven out by the other townspeople and took refuge in the storm drain and sewer systems beneath the Philadelphia ruins, where he stayed ever since.
• Still claiming to be a certified doctor, Smitty had given himself the unofficial and unwarranted title of local medic to the mutant populace. Though he was still capable of giving proper medical care and would have moments of lucidity, he seemed to just decide not to and preferred to make his own baseless treatments that often did more harm than good. This has garnered him a negative reputation and newcomers are often warned to not accept medical treatment from him, no matter how handsome he may be.

With the odd circumstances of his mutation, Smitty lacks the usual strength and endurance of a super mutant, but is more agile than a ghoul.

tab ■ What Once Was: Smitty speaks very little about his life before Sub-Philly. It's common for those infected with heavy amounts of FEV to forget who they were, but Smitty would occasionally make remarks that indicated that he still remembered who he once was. He seems convinced that there was something important that he needed to find in the old College of Physicians, where he claimed to have worked a long time ago.
tab ■ Damn It, Smitty!: Smitty has taken in a new "patient" he found stumbling around the Philadelphia wastes. After escorting them back to his clinic, he soon discovered, as his patient lunged for the throat of another, that she was in fact, a feral ghoul. Having escaped, the feral now stalked through Sub-Philly and Smitty feels responsible for the safety hazard he unleashed on the non-ghoul citizens. However, he does not want to kill the ghoul, but release her above ground, despite many others wanting to kill it on the spot.
