Well it depends on what part of the canon you want to follow. If you choose to disregard the Enterprise and Original series, this would simply be a racist comment. They pick on other species for having smooth ones, so... 
Now TOS Klingons looked like asian supervillains, and actually had smooth foreheads, so unless you take the Klingon storyline in the Enterprise canon, they don't make much sense and are a product of 1960's makeup techniques.
If you then take Enterprise's canon on board, saying a Klingon's mother has a smooth forehead is actually possibly truthful, due to the genetic manipulation and gene splicing with humans in a story arc explaining the smooth foreheads of TOS.

Uh I think you can google Klingon swearwords/insults. I usually just call people petaQ when I want to do a quick insult.
