Maybe some did die in hospitals. It's 19 deaths and 500 injured (more then they thought), 1 still in serious condition.
The ones at fault are the managers of the event and the city of Duisburg, who let 1,5 millions of people on an area which was only suitable for 250.000 people. It was infenced and the only way to get there was through tunnels, which had no emergency exit.
The heat and the narrowness was unbearable so many people tried to climb up a wall and some fell down this wall into the people, who where standing below. That caused a mass panic and the people crushed themselves. Most dead bodies and seriously injured where found right at the entrance of the tunnels.
