In real life, different mafia families have different territories under their control. Shop keepers and retailers of that area pay a certain "protection fee" to them, and in turn, mafia families promise not to ruin their businesses.
In this guild, territories operate mainly in the same manner, apart from the protection fees. Territories act as a symbol for the family's influence and power, so it would not be surprising that battles over territories take place every so often. Of course, territories are not marked by anything, so not to catch the attention of the police. The territories are kept sheerly through trust - and violence if necessary.
Disputes over territories can be settled by any means: negotiations between family bosses, all out fights between families, one on one fights, etc. In the end, territory disputes are settled between family bosses.
You can have territories within different cities, but you can only have them in two different countries. It won't make sense if you have a piece of land in Japan, USA, Korea and Italy. You can only expand your territories to a third country if your influence is strong enough, e.g. you've beaten all the other mafia families in your two countries.
You can't just barge into an area, kick some random person's a** and go "WOOT I HAS YOUR TERRITORY NOW SUCKAS". Nuuu, because that boss will come and kick YOUR a**. A territory dispute is ONLY solved by opposing mafia bosses.
