You must fill out this form and send it to either me or Xoneas, simple enough right?If you do not Xoneas will be swift to warn you once and most likely ban you the second time. If filled out incorrectly thewn you will have to redo it until we see fit. Answer truthfully, and be sure to ive links where it says.
