They usually have normal and then above and below it, hard and easy.

I usually pick normal, because I'm not sure how tough a games going to be, but there have been times, like in Resident Evil Remake where I regretted it. In games I've already played before like Metroid Trilogy, I play hard, because I'm too used to the game.
