Em hotep to all!
Is the only temple the House of Netjer in IL? I'm curious to know if any other temples exist off of the 'mother' temple...

I don't think there is, just the one in Chicargo, although I hope this expands as I live in England and would love to have a temple here. I'm sure one day, with more members and money, that this will be possible one day.
Your best bet is to ask Nisut (AUS) Herself.
