itachi didn't have a disease, he let sasuke win because he wanted sasuke to kill him, he killed the clan because it was a mission he was assinged by the hokage, he didn't kill sasuke because no matter what he could never hurt his little brother, just read shippend 43, last chapter tobi explains it all
