To be an Apprentice you need to talk to the gym leader of your choice there will be 8 gyms to choose from this is also your choice to be an apprentice for each gym
Heres my gym Apprentice Application forum for an Example
Inferno volcano gym:
To be one of my Apprentice you will need 6 fire types on your team and i must accept the Application for you to be 1 of my apprentice.

What do you know about EVs, IVs, and Natures?
