I realized why Ulquiorra put holes in his enemies in the same spot he has his hole. >.> Everyone else probably figured this out but I just realized it (when I reread the chapter where Ichigo dies).
Ulquiorra doesn't understand what a heart is. Naturally, the heart is in the middle of the chest- although the bulk of it is towards the left. His own hole is very near to where his heart should be. He strikes out at enemies and puts a hole where their heart should be. In one chapter (heh, we all know this chapter) he reaches out and touches Orihime's chest, where the heart is located. The place he strikes out at enemies. He even makes the remark (as he's touching her chest) that if he "rips open her chest, will he find [the heart]".
Ulquiorra may not know what a heart is but unconsciously he knows where it should be. I think someone hurt him a long time ago as a child and he now cuts himself off from everything. He no longer understands what it means to feel. But, at the same time, he strikes at everyone else's heart. Even though he doesn't realize that he does that.
Tying this altogether is his name. Ulquiorra means "The Boy Who Cries". Which, in a sense, is fitting and not fitting. His face shows streaks down his face that looks like tears. But he hardly shows any emotion.
.... Ok. Yes, this is my take on it. And the above is just a whole bunch of sudden theories by a fan. >-< Anyways.. take it seriously, don't take it seriously- I don't care. I am just posting what seems to be a finding. ^^
