I'd say perhaps Orihime, but she seems too shy for that. Although I could see a scene where Ichigo is injured, or more likely dying, and as Orihime's healing him, she kisses him, or holds his hand and tells him she loves him.
Then again, perhaps Ichigo, but in a slightly less romantic way. Instead of kissing her, he's more the type to hold her hand and smile, and tells her he has feelings for her.
Yes, I'm corny with my romance imaginings.

I agree with Crayon-san. Hime-chan is probably a little too shy to tell him when he's awake (she just barely got it out when he was asleep).
If Ichigo made the first move, it would be more subtle. Sort of like the scene after he defeated Byakuya; I could see the love confession to be more like a smile and an unspoken "I love you".
Actually, I can't see either of them saying "I love you" without being a couple for a while. The initial move would probably be some awkward, unspoken, yet mutual realization of the other's feelings. Then after some time, I can see Orihime confessing.
I'm not sure if that made sense, but I'm too lazy to change it. ^-^

I agree with Crayon-kun too, she's right about Orihime being shy. It also just isn't in Orihime's nature. I think Hime-chan wouldn't want to force her feelings on Ichi.
And we all know Ichi's petty dumb in the romance department, so I'm not sure what he would do.
In my opinion, it's only logical this situation calls for a middleman! (or middlewoman) What fun that would unlock. =D Seriously. Keigo could spread it around school, or Matsumoto-chan could try and set them up, or or or. There are lots of possibilities.
Since it came up, I really can't see Ichigo using the word love. I could see, when he realizes his feelings for Orihime, he'd try talking about it with Tatsuki or someone. I could kind of see him being all "It's about Orihime. I think I l-l-lo-lo. Damn it you know what I'm trying to say!" Stupid and random, but I could see it. sweatdrop

[SPOILER WARNING] (for people who arent up-to-date with the ANIME or Manga)
Ichigo will make the first move.. and my theory why he will;
It's sad but Ichigo isnt very preceptive. I'm pretty sure almost all of the Bleach characters know of Orihime's feelings for Ichigo. He has feelings for her, that's obvious (even if some people dont want to admit that). However it's possible it'll be an accident, just as it's possible that one of their friends will put them in a situation with each other and both will make the first move.
Orihime confessed even though he doesnt know it, though he knows she was there before she left for HM and that she healed his injuries (so he could go rescue her right away like his does) which was a direct violation of her orders from Ulquiorra. So the ball is really in Ichigo's court.
Then when Ichigo finally is reunited with Orihime (much thanks to Grimmy) he IMO makes the first move. Not a complete move mind you but it was a good first try. He'll try at it again I hope.
