How do you generally go about filling in noticeable Plot Holes? Namely those that are a major part of the plot.

I've recently picked up an older FanFic of mine from 2014. (Last chapter update was Sept 2014) It was one of the very first ones I made for the fandom.
While poking around for the new chapter I realized why I had fallen into such a long rut with it in the first place. And that was because of this one... generalized and rather noticeable plot hole.
Due to extremely poor planning, and beginner mistakes, I made a time gap of 12hrs between the character getting kidnapped and when the people come to help him. One of those "If you want to see them alive meet us at so and so time" situations.
At the time the victim was unconscious so I didn't think much of it and spent a few of the chapters going over what happened with the people who were going to aid him. I also ended up making it so the victim was only unconscious for maybe 4hrs tops, and that he had begun an attempt to escape.
Now said victim is not a "damsel in distress" type. He has been through a lot and is quite familiar with how to handle himself. This normally wouldn't be a problem... except I made the horrible choice to insert a time jump from him starting his escape in one chapter, to the arrival of his rescuers in the next.
This leaves me with an 8hr plot hole of which I have no idea what the victim is doing, has done, or what happened to him.... Let alone where to take it from there.
I don't know if I should somehow "patch" it together with some type of summary of his escape, in the next chapter, as he tries to meet up with his helpers? Ignore the plot hole all together and just roll with it as if it really didn't exist. (And hope the readers do as well)
Should I insert an extra chapter and re-write a large portion of the story, making the 12hrs into something more reasonable?
I could also have him simply appear during the confrontation and then have him explain what happened / how he escaped afterwards? (Also a kind of summary. "You didn't get to see the action but trust me. It happened.")
