Okay, I ran across this game in one of the groups I'm in on TinierMe. First someone says something they are going to do, and then the next person says how it goes wrong and ends it with some form of 'and then you die.'
For example!:
Persona 01: I'm going to eat a sandwich.
Persona 02: Ah, but it turns out that it was laced in arsenic... so you die.
Try and be creative, yah? Convolution is awesome. Okay, so I'll start:
I'm playing Final Fantasy XII.

Ah, but there was a short in the controller which shocked you to death... you die. xd

I went fishing in my small fish aquarium.
(Hm...maybe we should after someone does the "...and then you die." part then they have to make up a sentence so the game can continue?)

Ah.. yeah, you're supposed to say something you're doing after you kill the other person. I guess I forgot to say that. Also...
It turns out that some piranha sneaked into your aquarium and leapt out of the water to attack you. They decide you're tasty, so you get eaten... and, of course, die. o_o
I told off a mod. >_>

I went roller skating down the sidewalk in my shortshorts while eating vanilla ice cream.

*pokes Ia-kun* Play the game... you're supposed to say how I died for telling off a mod. (And, yes... I did kinda talk back to a mod, 'cause he moved my thread and ignored the suggestions I had written in it)
Also: some of the ice cream dripped onto your bare knee and you freaked out because it was so cold. You tried to wipe it off, but while you were distracted by that you skated into a twig, which tripped you up and sent you flying through the air into oncoming traffic. You were hit by a bus carrying the Canadian Olympic team and died.
I'm doing laundry.

You put on a shirt and the worms accidentally fell into your mouth. You die of worm infestation one month later. I'm touching myself at night. (lol)

Well, let's see. Since God kills a kitten everytime you masturbate, he made good on it. St. Francis, patron saint of animals, got fed up with all the kittens dying and decided to make an example out of you. He stormed into your bedroom and, using some of his super awesome Catholic Saint-ness voodoo, turned you into a kitten! Five seconds later some guy in Korea got goin' to a naked picture of Miku, and God struck you down... and you died.

While working diligently on your homework you come across a hard Math problem. You work at this problem for hours and hours sure you'll come up with the answer but the harder you worked on this problem the more the answer seemed to elude you. You eventually drive yourself insane and stab an eraser into your eye and of course you die... never solving the Math problem either.

*ponders* Okay, so you're grooving and dancing and having a grand ol' time. Unfortunately for you, somewhere else in the world d**k Cheney has finally finished his Deluxe Nuclear Missile Kit (it took him years to figure out that tab A fit into slot A...). He isn't quite sure how to work the navigation system, though, so he just hits a lot of random buttons until the launch sequence activates.
So, back to you and your groovy self: The padded room is hot and the air conditioner is on the blink. You're hot and sweaty, but you don't care! You do a super spining move and moisture goes everywhere! (Ew.) Sadly, some of it lands on a crack in the wiring and a fire breaks out! The padding is for some reason super flammable and the fire spreads like nobody's business! You're trapped. Oh, noes!
Meanwhile, the Cheney Missile is hurtling towards your exact location! Suddenly there is a HUGE 'SHROOM OF DEATH where your room used to be!
Unfortunately, the coronor's office never could find your body, so we aren't sure whether you died from the fire or the bomb. Either way, you died.
In other news Cheney was thrown into a pit under a farm full of super gassy cows as punishment.

You are making the greatest sandwich known to man. You pull out your plate and are barely missed getting knocked stupid by all the falling plates. You jump back to avoid the flying sharp shards of the broken plates. You decide to leave the broken bits for later clean up.
You then head to the fridge to pull out all those great ingredients that your sandwich will need to be awesome. The fridge though unbenounced to you has a short and has become electrified. You grab the handle to pull and are shocked, which sends you flying into the cupboards, you fall to the ground onto the shards of broken plate, but are still alive. You reach up to pull yourself up to call for help, but you knock the butter knife down and it plunges into your eye and you die.
