BTW, #11 has two correct answers.

One would be Aina and the Apsalus II, even though she didn't actually damage Shiro's Gundam directly, IIRC (It was damaged saving her MA from sliding off of a cliff.)

The second answer is Ginias and the Apsalus III, which is what I answered with. The damage involved was the entire lower half of the Ez8 being burned away by the misfiring mega particle cannon.
