well it could go either way i think.
sakura would still be his fan girl at first and hed still be an emo b*****d.
why?
because you forget sasuke and itachis father and a tight assed sadistic over expectant b*****d.
and if itachi some how managed to not kill him and the rest of the clan. sasuke would turn out like itachi or he would become the mass murderer, or just kill his father.
but sakura would meet itachi in some way. She becomes a medic right? well dont you think ANBU get hurt? Being on teh same team as sasuke means she would go to his house sometime, and he might coincidentally be at home as well and then sasuke would be forced to introduce them.
but sasuke could also turn out normal, and then sakura might not have a crush on him. and then she might loose all intrest in teh uchihas period and go after naruto or neji.

And yet another theory hits me...
Sasuke viewed Itachi as a rival, yet best friend, before the clan was killed off, right? Well, if the clan was never killed off in the first place, he might still view him as competition. And because Sasuke wouldn't be all brooding and avenging, he wouldn't be as attractive and popular since the newage (Naruto-wise) trend for attractive guys are the mysterious, dark kinds.
And Itachi would probably attract the most attention from all ages because he's the darker, more mature one of the two. So maybe Itachi would be the one with the fangirls, the rivalry of his younger brother, and the attractive appearance. I have a feeling that he'd be secretly pleased with the look as well.
But... like we said earlier, it's all up to Kishimoto.

Hmm, I just thought of this....
If Itachi never left...then maybe the whole plot of Naruto would change! I mean, one of the reasons Sasuke left for Oro was because to gain power and avenge his dead clan...and since it's not dead...then I wonder what would happen to the series??? Any suggestions anyone?

1. hmmmm if itachi didnt kill off his clan then he might end up meeting sakura since she tusnades apprentice and they might go on missions together..... hmmm this is very interesting.....
2. sasuke might find sakura after a mision hurt and alone i the woods and bring her to his place to cheack her to see if shes ok then itachi might happen to come home and find them...
3. itach might see sakura and sasuke spar and take an interest in her.... idunno!!
