This story is confusing me.... Why would the doctor reconstruct his face as a woman if he obviously saw he was male?

I have no idea. Rando wakes up just in time to prevent the doctor from taking away his manhood, and several times the Doctor try to get him to complete his transformation.

I dont know, some combination of learning disability/huge balls (the doctors)

The doc actually said that "Some guys have really feminine faces"

Because he wanted to, and found ways to rationalize it in his own head. The only reason he doesn't cosmetically alter Rando further is because Rando won't let him.
